The phrase ‘person‐centred care’ (PCC) reminds us that the fundamental philosophical goal of caring for people is to uphold or promote their personhood. However, such an idea has translated into promoting individualist notions of autonomy, empowerment and personal responsibility in the context of consumerism and neoliberalism, which is problematic both conceptually and practically. From a conceptual standpoint, it ignores the fact that humans are social, historical and biographical beings, and instead assumes an essentialist or idealized concept of personhood in which (...) a person is viewed as an individual static object. From a practical standpoint, the application of such a concept of personhood can lead to neglect of a person's fundamental care needs and exacerbate the problems of social inequity, in particular for older people and people with dementia. Therefore, we argue that our understanding of PCC must instead be based on a dynamic concept of personhood that integrates the relevant social, relational, temporal and biographical dimensions. We propose that the correct concept of personhood in PCC is one in which persons are understood as socially embedded, relational and temporally extended subjects rather than merely individual, autonomous, asocial and atemporal objects. We then present a reconceptualization of the fundamental philosophical goal of PCC as promoting selfhood rather than personhood. Such a reconceptualization avoids the problems that beset the concept of personhood and its application in PCC, while also providing a philosophical foundation for the growing body of empirical literature that emphasizes the psychosocial, relational, subjective and biographical dimensions of PCC. Throughout the critical period Kant enigmatically insists that reason is a ‘unity’, thereby suggesting that both our theoretical and practical endeavors are grounded in one and the same rational capacity. How Kant's unity thesis ought to be interpreted and whether it can be substantiated remain sources of controversy in the literature. According to the strong reading of this claim, reason is a ‘unity’ because all our reasoning, including our theoretical reasoning, functions practically. Although several prominent commentators endorse this view, it (...) is widely thought to lack exegetical support. This paper seeks to strengthen the case for this reading by showing how theoretical reason's positive function, as Kant presents it in the Appendix to the Transcendental Dialectic, may be construed as fundamentally practical. I argue that reason's supreme regulative principle ought to be understood as a categorical practical imperative. This interpretation, I suggest, resolves the apparent inconsistencies that blight Kant's account of the principle in the Appendix, while bringing greater overall coherence to his account of theoretical reason's regulative function. In this paper, I explore some of the ambivalent potential of Graham Harman’s post-humanist object-oriented ontology for thinking about human beings as objects, and for how to be with human beings as objects. In particular, I consider the work of feminist phenomenologists attuned to objectification as both having a tradition of object-orientation and as already contesting the idealism that Harman opposes. Objectified human beings inhabit a site of ontological duality, often knowing themselves as objects for others, who thus experience the (...) ontological and epistemological disruptions that can emerge from the human activity of objectification. Absence of these analyses in OOO constitutes an important oversight, since the analyses draw attention not only to object relations among human beings, but thereby also point to ways of understanding human concept relations with non-human objects. In this essay, I explore the ways that Beauvoir’s description of philosophical novels reveals her understanding of consciousness as a particular sort of ambiguity: that which not only gives the world meaning, but which also, necessarily, finds meaning in the world through the values, ideas, and objects given to it by others. It is through the philosophical (metaphysical) novel that Beauvoir finds a medium for the philosophical communication of ambiguity – that is, a medium for writing human being. More specifically, (...) I consider the metaphysical stance Beauvoir is able to describe because of her commitment to philosophical literature. In writing, and in reading, fiction, what is manifest is both found and given, discovered and created; and the metaphysics of the novel offers a way to read philosophy as poeisis, poetry in the sense of bringing-forth or revealing worldly meaning, in ways that are ambiguously particular and universal. The complex emotional work of nurses calls for more recognition of emotional labour and the incorporation of emotional labour in nursing education. Based on participant observation and semistructured interviews, we describe the experiences of student nurses in two nursing homes for elderly people with dementia in the Netherlands. We analyse their interactions using Goffman's dramaturgical view on the front and backstage behaviour and the distinction between surface acting and deep acting. The study reveals the complexity of emotional labour, as nurses (...) swiftly adapt their communication styles and behavioural strategies between settings, patients, and even between moments within one interaction sequence, which shows that the theoretical binaries fail to fully capture their skills. Although student nurses take pride in their emotionally taxing work, the societal undervaluation of the nursing profession negatively impacts their self‐image and ambitions. More explicit recognition of these complexities would enhance their self‐appreciation. This calls for a professional ‘backstage area’ that allows nurses to articulate and strengthen their emotional labour skills. Educational institutions should provide this backstage for nurses‐in‐training to strengthen these skills as part of the professional skill set. (shrink)

Feminist and post-colonial epistemologists, philosophers of science, and thinkers more generally may find themselves in a distinct form of difficult situation regarding their access to and authority over knowledge within the academic world. Because feminist and post-colonial approaches to knowledge require an acute awareness of relations of domination and the ways in which these pervade the social and epistemic world, it is often difficult to know how to proceed in making theory. These theorists are in particularly ripe positions to benefit (...) from what philosopher-physicist Karen Barad offers us. In this paper, I engage with parts of Karen Barad’s theory of agential realism, both critically and self-reflexively. I assert that allowing Barad’s theory to inform and structure our thinking and language makes knowers better able to meet certain requirements of epistemological responsibility, particularly with regard to the ways we make theory. Moreover, I attempt to assert this in a way that is mindful of how her theory speaks to and accounts for my doing so. (shrink)

Catherine Elgin proposes a novel principle for identifying epistemic virtue. Based loosely on Kant’s Categorical Imperative, it identifies autonomy as our fundamental epistemic responsibility, and defines the epistemic virtues as those traits of character needed to exercise epistemic autonomy. I argue that Elgin’s principle fails as a criterion of epistemic virtue because the instrumental conception of autonomy on which it relies leads to an untenable relativism. Despite this, I suggest that autonomy may yet furnish a plausible criterion for epistemic virtue, (...) provided we construe autonomy as Kant does, as grounded in categorical rather than instrumental reason. In this article, we examine the ways in which the notion of interdisciplinarity was understood, implemented and experienced by researchers at a government-funded Chilean climate research centre. Our multi-site ethnography, consisting of interviews, participant observations, and document analysis, was motivated by three key aims. First, to generate an inductive, multi-faceted picture of the lived meaning of “interdisciplina” at the Centre; second, to explore whether and to what extent the “peripheral” features of the research context would exacerbate the challenges associated with (...) practicing interdisciplinarity, and third, to see whether frictions between disciplines at the Centre could be considered productive “dissonances” in Stark’s sense of the term. We found that despite the centre efforts to produce a common framework to regulate interdisciplinary research, its researchers nevertheless understood, enacted and experienced it in diverse ways. More specifically, we found that researcher´s conceptions of interdisciplinarity were coloured by their lived experiences of attempting to practice it, and in particular by the benefits and costs they associated with doing so. This in turn was linked to several variables, including the specific balance between disciplines, the absence or presence of shared, clearly-defined goals, the affirmation of a common research ethic or motivational commitment, and the structural-material conditions of the research in question. We also found that the research conditions characteristic of the Global South do tend to exacerbate the well-documented challenges associated with interdisciplinarity, yet that the adversities associated with precarious conditions were often met by increased resilience and bonding among researchers, who use creative and collaborative strategies to adapt to adversity. In this paper, I draw on the mutually implicated structures of tragedy and self-formation found in Hegel’s use of Sophocles’ Antigone in the Phenomenology. By emphasizing the apparent distinction between particular and universal in Hegel’s reading of the tragedies in Antigone, I propose that a tragedy of action (which particularizes a universal) is inescapable for subjectivity understood as socially constituted and always already socially engaged. I consider universal/particular relations in three communities: Hegel’s Greek polis, his community of conscience, and my (...) reading of certain feminist communities. The position I propose establishes a ground from which to approach subjects, and implies that all subjects may be understood as the result of relations embodying potential tragedy. This speaks to contemporary concerns about marginalization, identity articulation, and relations of recognition. (shrink)

In this paper, I take up Graham Harman’s critique of the philosophy of access as well as his proposed non-anthropocentric ontology, and I ask what it would be like for human beings to live or practice such a proposal. Drawing on Harman’s thinking about prehension, but shifting focus towards work in critical phenomenology and feminist science studies, I argue for the importance of human prehensive self-awareness within non-anthropocentric ontological practices, an awareness that emerges phenomenologically and in practice. Extending both Donna (...) Haraway’s theory of companion species and phenomenological practices of being-here with other things, I lay a groundwork for practicing being human as companion object. Since it is relatively uncontentious that a capacity for raw sensation precedes and underpins all more sophisticated mental capacities, our question is tantamount to asking when human beings first have experiences with sensational content. Two interconnected features of our argument are crucial. First, we argue that experiences with sensational content are supervenient on facts about electrical activity in the (...) cerebral cortex which can be ascertained through EEG readings. Second, we isolate from other notions of a‘functioning brain’that which is required to underpin the view that a cortex is functioning in a way which could give rise to rudimentary conscious experiences. We investigate the development in the human fetus of the anatomical and chemical pathways which underpin cortical activity and the growth and maturation of the electrical circuitry specifically associated with sensational content in adult experience. We conclude that a fetus becomes conscious at about 30 to 35 weeks after conception; an answer based on a careful analysis of EEG readings at various stages of cortical development. Finally, we survey the possible ethical ramifications of our answer. (shrink)

Next SectionBackground: Physicians face ethical difficulties daily, yet they seek ethics consultation infrequently. To date, no systematic data have been collected on the strategies they use to resolve such difficulties when they do so without the help of ethics consultation. Thus, our understanding of ethical decision making in day to day medical practice is poor. We report findings from the qualitative analysis of 310 ethically difficult situations described to us by physicians who encountered them in their practice. When facing such (...) situations, the physicians sought to avoid conflict, obtain assistance, and protect the integrity of their conscience and reputation, as well as the integrity of the group of people who participated in the decisions. These goals could conflict with each other, or with ethical goals, in problematic ways. Being aware of these potentially conflicting goals may help physicians to resolve ethical difficulties more effectively. This awareness should also contribute to informing the practice of ethics consultation. Objective: To identify strategies used by physicians in dealing with ethical difficulties in their practice. Design, setting, and participants: National survey of internists, oncologists, and intensive care specialists by computer assisted telephone interviews (n = 344, response rate = 64%). As part of this survey, we asked physicians to tell us about a recent ethical dilemma they had encountered in their medical practice. Transcripts of their open-ended responses were analysed using coding and analytical elements of the grounded theory approach. Main measurements: Strategies and approaches reported by respondents as part of their account of a recent ethical difficulty they had encountered in their practice. Results: When faced with ethical difficulties, the physicians avoided conflict and looked for assistance, which contributed to protecting, or attempting to protect, the integrity of their conscience and reputation, as well as the integrity of the group of people who participated in the decisions. These efforts sometimes reinforced ethical goals, such as following patients’ wishes or their best interests, but they sometimes competed with them. The goals of avoiding conflict, obtaining assistance, and protecting the respondent’s integrity and that of the group of decision makers could also compete with each other. Conclusion: In resolving ethical difficulties in medical practice, internists entertained competing goals that they did not always successfully achieve. Additionally, the means employed were not always the most likely to achieve those aims. Understanding these aspects of ethical decision making in medical practice is important both for physicians themselves as they struggle with ethical difficulties and for the ethics consultants who wish to help them in this process. Background: Ethics support services are growing in Europe to help doctors in dealing with ethical difficulties. Currently, insufficient attention has been focused on the experiences of doctors who have faced ethical difficulties in these countries to provide an evidence base for the development of these services.Methods: A survey instrument was adapted to explore the types of ethical dilemma faced by European doctors, how they ranked the difficulty of these dilemmas, their satisfaction with the resolution of a recent ethically difficult case (...) and the types of help they would consider useful. The questionnaire was translated and given to general internists in Norway, Switzerland, Italy and the UK.Results: Survey respondents ranged in age from 28 to 82 years, and averaged 25 years in practice. Only a minority reported having access to ethics consultation in individual cases. The ethical difficulties most often reported as being encountered were uncertain or impaired decision-making capacity , disagreement among caregivers and limitation of treatment at the end of life . The frequency of most ethical difficulties varied among countries, as did the type of issue considered most difficult. The types of help most often identified as potentially useful were professional reassurance about the decision being correct , someone capable of providing specific advice , help in weighing outcomes and clarification of the issues . Few of the types of help expected to be useful varied among countries.Conclusion: Cultural differences may indeed influence how doctors perceive ethical difficulties. The type of help needed, however, did not vary markedly. The general structure of ethics support services would not have to be radically altered to suit cultural variations among the surveyed countries. (shrink)

In this book, S. A. Lloyd provides a radical interpretation of Hobbes' laws of nature, revealing them to be not egoistic precepts of personal prudence but rather moral instructions for obtaining the common good. This account of Hobbes' moral philosophy stands in contrast to both divine command and rational choice interpretations. Drawing from the core notion of reciprocity, Lloyd explains Hobbes' system of 'cases in the law of nature' and situates Hobbes' moral philosophy in the broader context of his political (...) philosophy and views on religion. Offering ingenious new arguments, Lloyd defends a reciprocity interpretation of the laws of nature through which humanity's common good is secured. (shrink)

In an international survey of rationing we have found that European physicians encounter scarcity-related ethical difficulties, and are dissatified with the resolution of many of these cases. Here we further examine survey results to explore whether ethics support services would be potentially useful in addressing scarcity related ethical dilemmas. Results indicate that while the type of help offered by ethics support services was considered helpful by physicians, they rarely referred difficulties regarding scarcity to ethics consultation. We propose that ethics consultants (...) could assist physicians by making the process less difficult, and by contributing to decisions being more ethically justifiable. Expertise in bringing considerations of justice to bear on real cases could also be useful in recognising an unjust limit, as opposed to a merely frustrating limit. Though these situations are unlikely to be among the most frequently referred to ethics support services, ethics consultants should be prepared to address them. (shrink)

S. A. Lloyd proposes a radically new interpretation of Hobbes's Leviathan that shows transcendent interests - interests that override the fear of death - to be crucial to both Hobbes's analysis of social disorder and his proposed remedy to it. Most previous commentators in the analytic philosophical tradition have argued that Hobbes thought that credible threats of physical force could be sufficient to deter people from political insurrection. Professor Lloyd convincingly shows that because Hobbes took the transcendence of religious and (...) moral interests seriously, he never believed that mere physical force could ensure social order. Lloyd's interpretation demonstrates the ineliminability of that half of Leviathan devoted to religion, and attributes to Hobbes a much more plausible conception of human nature than the narrow psychological egoism traditionally attributed to Hobbes. Environmentalist philosophers often paint a holistic picture, stressing such things as the continuity of humanity with wider nature and our membership of the 'natural community' . The implication seems to be that a non-anthropocentric philosophy requires that we strongly identify ourselves with nature and therefore that we downplay any human/non-human distinction. An alternative view, I think more interesting and plausible, stresses the distinction between humanity and a nature valued precisely for its otherness. In this article I discuss some of its (...) main elements, and some of the difficulties involved with keeping nature's otherness in focus. Firstly (in sections 1-5), I try to clarify what I take to be the otherness-based position by distinguishing it from the apparently similar views of John Passmore, Robert Elliott and Keekok Lee, and some opposed holistic views, especially of J. Baird Callicott. Then, in the second half of the article (sections 6-7), I argue that if nature is valued in virtue of its otherness, this value is best thought of as an extrinsic, final and objective good, where 'objectivity' is a 'method of understanding', in Thomas Nagel's sense. Although I give some reasons for preferring an otherness account to certain alternative positions, I make no overall attempt to 'prove' that nature is valuable for its otherness. My aim is to show that, if it is, then this seems the best way to understand that value. (shrink)
